Example of patient care
We know a patient who is 80 years of age, a widow and who lives alone. She is able to take care of herself, wash and cook, but cannot easily travel outside her house. Her shopping, cleaning and laundry are performed by her son, who lives in the same neighborhood. The son works full time.
The patient has diabetes, high blood pressure, an under-active thyroid gland and a weak heart. She uses a walker.
This patient needs to be seen on a regular basis, but her trips to the doctor’s office are a major expedition. In addition, the visits need to fit the son’s schedule and he is busy with his own life and family.
What are her options? Until now, all she could do is pack up and go to her doctor’s office. Now she can be visited in her own home.
- The son does not have to take time off from work to accompany her to the office.
- She can be observed in her in her own environment which allows the doctor to cutomize her care plan.
- It is less stressful for the patient, giving her and the family peace of mind.
- The doctor can catch any new developments early before they become severe enought to require hospitalization.
- It is easier to maintain independence and functionality.
Advantages of seeing patients in their own environments
